Carer Survey

Dartford & Gravesham Trust is committed to providing high quality care to people admitted to hospital with dementia and/or delirium. It is essential to involve carers and take into account their expert knowledge of the person with cognitive impairment in order for our staff to provide person-centred care.

This survey is about whether or not, as a carer, you felt supported by Darent Valley Hospital. By reviewing the responses, we hope to identify what we do well and where we need to improve.

If you are a carer for a patient with dementia, suspected dementia or delirium, we would be grateful if you would complete this short questionnaire. Taking part is voluntary and responses remain anonymous.

Please remember, this questionnaire is about the patient's current visit to Darent Valley Hospital.

If you have any queries about the questionnaire, please ask the nurse looking after the patient.

In addition to this questionnaire, please ask nursing staff for a 'This is me' document to complete; this will be kept at the end of the patient's bed and used by staff to familiarise themselves with the life story, personal preferences and wishes of the patient to ensure patient centred care.

Concerns or worries - if you would like support or have any questions related to cognitive care whilst the person you look after is in the hospital, please speak to the nurse in charge of the ward. There is also a Dementia Specialist Nurse available Monday to Friday who can be contacted on your behalf by the nurse in charge.

Dementia Buddies - Darent Valley Hospital has trained Dementia Buddy volunteers working on specific wards to engage positively with patients with dementia. Volunteers provide social interaction, assistance with mealtimes and encourage patients to engage in activities to help maintain their cognitive abilities.
