Chaplaincy Survey

This is a survey for patients, relatives and carers to tell us about their experience with the Chaplaincy team at York and Scarborough Teaching Hospitals NHS Foundation Trust.

While you have been in hospital you saw a chaplain. This questionnaire is designed to help us understand how the chaplain helped you. Your experience of our service is valued, and your feedback will be used to identify improvements we can make to our services to better support patients, relatives and carers.

Your answers will be anonymous (unless you choose to answer Question 7), confidential, and your participation is voluntary. Any data you provide will only be used in accordance with the Trust Privacy Policy .

The survey will take approximately 5 minutes to complete.
